Before renting a trailer, download, complete, and email the required forms below.
What you need to send
You must complete and send three forms:
- The Credit Card Authorization Form — required for everyone.
- The Credit Card Authorization – Toll Charges form — required for everyone.
- One Credit Application — the Personal one if you're renting as an individual, or the Business one if you're renting under a company.

Important: Both credit card authorization forms (including Toll Charges) and one Credit Application (Personal or Business) must be completed and emailed before you can rent a trailer.
